Description
Snohomish County Human Services, Long-Term Care and Aging, is soliciting proposals from organizations to provide countywide Family Caregiver Support Program services for unpaid caregivers. The County expects to award at least two contracts. Required services include information and assistance, Tailored Caregiver Assessment and Referral (TCARE) screening and assessment, individualized care planning, support groups, caregiver training and education, counseling, respite care, supplemental services such as housework, errands, grab bars, and incontinence supplies, health and wellness referrals, outreach, interagency coordination, and monthly client and service reporting through the Community Living Connections GetCare system. TCARE assessments include home visits, and each successful bidder must have at least one certified TCARE assessor by the contract start date. The initial contract period is July 1, 2027, through June 30, 2028, with the possibility of annual extensions or new contracts for up to four additional years. Estimated total funding for all contracts is $664,655 annually, subject to appropriation, with a required 25 percent non-federal match. Proposals must address technical, management, and budget sections and be submitted as a single PDF in 12-point Arial font. The proposal must include the face sheet, technical section, management section, budget worksheet, and required organizational and financial documentation listed in the solicitation.
