Description
The City of Davenport is soliciting bids for repairs to deteriorated concrete at the Duck Creek Golf Course maintenance building. The base work includes shallow and regular concrete repairs, including removal of damaged concrete, cleaning and preparation of surfaces, reinforcing and embedded elements, forming, placement of repair concrete, and all associated labor, materials, equipment, and mobilization. The project includes approximately 60.5 square feet of shallow repair and 139.2 square feet of regular repair. An optional alternate requests cleaning, surface preparation, and application of a non-cementitious concrete coating to approximately 1,500 square feet of garage floor. The plans specify concrete repair procedures, reinforcement and concrete requirements, submittals for reinforcement shop drawings, concrete mix designs, embedded steel shop drawings, and concrete-coating manufacturer data. The contractor must also provide a current certificate of insurance, complete the Bidder Status Form, and submit the DBE subcontractor contact forms with required good-faith-effort documentation. For bids over $100,000, a 5% bid bond is required; the awarded contractor must provide 100% performance and payment bonds and a four-year maintenance bond.
