Description
Monroe County is soliciting proposals for a contractor to operate local adult re-entry coordination and case-management services under the New York State County Re-entry Task Force model. The selected contractor must provide a full-time re-entry coordinator and case-management support for approximately 325 moderate- to high-risk individuals and designated special populations returning from state prison. Services include individualized service planning; assistance with public benefits, employment, housing, transportation, clothing, prescriptions, education, and treatment; appointment reminders and transportation when necessary; family meetings; and coordination with parole, public agencies, and community service providers. The contractor must also support strategic planning, community education and outreach, bi-monthly task-force meetings, bi-weekly case conferencing, and required monthly and quarterly reporting. The program is performance-based, with annual targets including 325 intake assessments, 300 participants retained for 45 days, 55 enrollments and 30 completions each for an approved employment-readiness program and cognitive-behavioral intervention. Respondents must submit a detailed work plan and budget, organizational and experience information, key-personnel resumes, required certifications, an insurance certificate, and MWBE/SDVOB utilization materials. The agreement is anticipated to have a one-year base term with four additional one-year renewal options.
