Description
The West Virginia Department of Health is soliciting proposals from eligible nonprofit organizations, coalitions, and units of local government to implement the fifth edition of the Making Proud Choices evidence-based curriculum and related pregnancy, HIV, and sexually transmitted infection prevention activities for high-risk and vulnerable youth ages 10–19. Eligible populations include youth in or aging out of foster care, homeless youth, youth with HIV/AIDS, victims of human trafficking, pregnant or parenting youth under age 21, minority youth, and youth in areas with high teen birth rates. Selected sub-awardees must provide programming emphasizing abstinence and contraception education, address at least three mandated adulthood-preparation subject areas, participate in specified health events, maintain enrollment and attendance records, complete fidelity and evaluation activities, collect entry and exit survey data, and submit semiannual performance reports. Applications must include a proposal narrative, detailed line-item budget, budget narrative, target funded budgets, a signed Subrecipient Information Form, and a grantee contact list. The funding period is October 1, 2026, through September 30, 2027, with potential annual renewal based on performance and program success.
