Description
The affidavit is for a bidder or authorized representative to certify that the bid is genuine and not collusive or sham. It requires affirmations that the bidder has not coordinated with other bidders, fixed prices or cost elements, sought an improper competitive advantage, shared bid information with potential bidders, or submitted false statements. The affidavit also requires confirmation that no county employee has a financial interest in the bidder or the bid. The form includes signature and title fields for the affiant and requires notarization, including the notary's signature, seal, and commission expiration information.
