Description
The City of Toledo is requesting a lump-sum quote for labor, equipment, and materials to restore concrete and asphalt cuts following water-main and water-service repairs at 22 listed locations in Lucas County. The listed work includes approximately 307 square feet of asphalt repairs and 579 square feet of concrete repairs. Work must meet City of Toledo and Ohio Department of Transportation specifications, and the contractor must maintain traffic, secure work sites, restore disturbed areas, remove debris, and provide completion photographs. A valid City of Toledo Sidewalk Contractors License is required for the prime contractor and any subcontractors. Prevailing-wage requirements apply, and the contractor must submit certified payroll reports during performance. The bid requires a 5% bid guaranty, a signed construction contract, a tax-exemption certificate, and the specified project paperwork. The contract duration is one year; the contract form also sets completion deadlines after notice to proceed. A pre-construction meeting is required before work begins. Payment and performance bonds and a project labor agreement are required if applicable under the stated thresholds and conditions.
