Description
The City of Sierra Vista is soliciting proposals from qualified medical providers to conduct physical examinations for prospective and current employees. These services include post-job offer/pre-employment physicals, occupational health medical services, and Arizona Department of Transportation Commercial Driver's License examinations. Advice and consultation services related to Occupational Health will be provided to the City at no additional cost. Providers must be located within Cochise County and licensed to provide medical services in Arizona. For CDL physicals, medical professionals must be listed as certified medical examiners in the National Registry of Certified Medical Examiners (NRCME). Results for DOT physicals must be submitted to the ADOT online portal within 24 hours. The contract term is for two years, with the option to renew for three additional one-year periods, not to exceed five years. The City cannot guarantee quantities, as services will be on an as-needed basis. The City intends to contract with one or two providers, one as primary and a second as secondary.
