Description
The Department of Veterans Affairs (VA) is issuing a Request for Quotation (RFQ) for reference laboratory services for the Phoenix VA Healthcare System. This solicitation aims to establish a single-award, Firm-Fixed Price, Indefinite-Delivery, Indefinite-Quantity (IDIQ) contract. The services required include a full range of clinical and anatomic pathology diagnostic testing, specimen preparation and storage, transportation of specimens, and consultative services. The contractor must be CLIA certified and meet all requirements outlined in the Statement of Work (Attachment B), including specific licensure and accreditation standards. This IDIQ contract will have a one-year base period with four one-year option periods. The guaranteed minimum award amount is $500.00, with a maximum aggregate value of $4,250,000.00. Pricing for all services, including labor, supplies, and transportation, is to be provided on a per-test basis as detailed in the Price Schedule (Attachment A). The contract requires the contractor to maintain specific quality assurance programs and provide customer service accessible via telephone during business hours. All submissions must adhere to the specific instructions and deadlines provided, including acknowledgment of amendments and submission of required company information and special licensure evidence.
