Description
The Commonwealth of Kentucky, through the Finance and Administration Cabinet and Kentucky Division of Oil and Gas, is soliciting proposals to plug, remediate, and reclaim 21 orphan oil wells in Ohio County. Work includes site preparation and access-road work; removal of wellhead equipment, flowlines, meters, separators, and debris; downhole inspection and obstruction removal; cement plugging; casing cut-off; handling of potentially contaminated soil and TENORM; and restoration of wellsites and access roads using erosion-control, drainage, stabilization, and revegetation best management practices. Sites must maintain permanent vegetative cover and effective drainage for one year before final acceptance and payment. The contractor must provide qualified personnel, committed equipment, a detailed work plan, project history, past-performance information, subcontractor information, and Davis-Bacon compliance procedures. All wells must be plugged within 180 days of contract award, with substantial completion required within 365 days and final completion one year after substantial completion. A 5% bid guarantee is required, along with 100% performance and payment bonds from the successful bidder. Responses must include the signed solicitation, latest signed addendum if applicable, cost proposal and technical solution, required affidavit, transmittal letter, applicable tax and foreign-entity documentation, and the specified experience, equipment, work-plan, past-performance, Davis-Bacon, and subcontractor forms.
