Description
The Indiana Department of Administration, on behalf of the Indiana Family and Social Services Administration's Division of Mental Health and Addiction, is soliciting proposals from one or more contractors to provide behavioral health ombudsman services. The detailed statement of work is provided in Attachment K. Proposals must include an Executive Summary, Business Proposal, Technical Proposal, Cost Proposal, Cost Proposal Narrative, Cost Assumptions, Conditions, and Constraints, Attestation Form, and the other required attachments and templates identified in the solicitation. The contract term is two years from contract execution, with up to three one-year renewals at the State's option. Evaluation includes mandatory compliance, management and technical quality, cost, Indiana business or small-business participation, and Indiana Veteran Owned Small Business subcontractor participation. The solicitation establishes a 3% Indiana Veteran Owned Small Business subcontracting goal and permits evaluation points for qualifying participation.
