Description
The City of Batavia is seeking a qualified healthcare or telehealth provider to deliver a community-wide program for eligible residents and participating businesses, regardless of health insurance status or age. The program must support up to 10 individuals per household or business subscription, use address-based enrollment without requiring a household census, provide access without requiring a mobile application, and offer text, telephone, and/or video care through appropriately licensed healthcare professionals. The provider must offer routine, non-emergency telehealth services, prescription-related services, referrals to in-person or emergency care, and discounted or transparent self-pay pricing for laboratory testing and diagnostic imaging, including bloodwork, X-rays, CT scans, MRIs, and ultrasounds. The program must integrate with the City's municipal utility billing system with no administrative, implementation, billing, integration, technology, software, or transaction costs to the City. Proposals must address privacy and HIPAA compliance, implementation, communications, reporting, pricing, community or municipal experience, and provide at least three references. The provider must also complete the mandatory compliance matrix and vendor question.
