Description
The U.S. Department of Veterans Affairs is seeking a complete, all-or-nothing quote for dental instruments and related supplies to support dental services at a new clinic. The attached pricing spreadsheet lists 112 line items, including dental mirrors, explorers, excavators, pluggers, carvers, rubber dam equipment, elevators, extraction forceps, retractors, curettes, scalers, scissors, hemostats, rongeurs, tissue and needle-holding instruments, and instrument cassettes. Quotes must include completed pricing, product specifications or cut sheets identifying the offered brand and model and demonstrating compliance with the salient characteristics, expected delivery date, Buy American/Trade Agreements certification, the applicable limitations-on-subcontracting certification, and an OEM authorization letter when the offeror is not the manufacturer. Freight must be included in CLIN 0001, delivery is FOB destination, and the quote must be submitted by email with a 4 MB limit. The procurement is a 100% Service-Disabled Veteran-Owned Small Business set-aside.
