Description
Monroe County is seeking a contractor to provide Medicaid coverage and application services for residents of Monroe Community Hospital, a 566-bed skilled nursing facility. The selected contractor will assist residents and families with nursing home Medicaid applications, eligibility assessment, document collection, application submission, agency follow-up, retroactive coverage coordination, and conversion from Community Medicaid to Chronic Care Medicaid when applicable. The scope also includes monitoring recertification timelines, notifying families and nursing home staff of upcoming requirements, assisting with recertification forms and documentation, maintaining secure HIPAA-compliant records, tracking applications and outcomes, and providing regular progress reports to the nursing home Finance Officer. The agreement is expected to have a one-year base term with four additional one-year renewal options. Proposals must include organizational information, relevant experience, key personnel resumes, a project narrative and work plan, cost proposal, certifications, insurance documentation, exceptions, and MWBE/SDVOB utilization materials.
