Description
Louisville Metro Government is soliciting proposals for a 12-month price contract covering the manufacture and installation of logos and decals on Emergency Services vehicles, including ambulances, Ford Explorers, and Ford Escapes. The price sheet requests unit pricing and completion times for each vehicle type. Proposals must address manufacturing and installation turnaround time, warranty length for labor and materials, the number of working days between jobs, pricing, and a sample decal using 3M products for quality evaluation. Required submission materials also include vendor information, acknowledgements and preferences, a W-9, Ban the Box compliance certification, Good Faith Effort documentation, and an IT Risk Assessment Questionnaire or SOC Type 2 Audit Report. The contract may be renewed annually, with a maximum total contract period of five years.
