Description
The Defense Logistics Agency is soliciting a quotation for one ophthalmic depth perception apparatus, NSN 6540-01-486-3725. The apparatus must identify vision problems and support house fly stereopsis, grade circle, animal, and Stereo Fly tests for children. The unit of issue is one each, and the bidder must identify the company and part number supplied. The item is subject to FDA regulation and must be packaged and marked in accordance with the stated medical packaging and marking requirements. Labels must include the NSN, manufacturer and CAGE information, manufacture, expiration, or retest date, and contract or lot number; containers must include the lot-control number and manufacture date. Alternate offers require complete technical data describing both the requested and alternate products.
