Description
Atlantic County is soliciting proposals for a vision care benefits program for approximately 1,885 employees, retirees, and eligible dependents. The program must provide annual comprehensive eye examinations, eyeglasses or qualifying spherical soft daily-wear contact lenses, visual analysis, dispensing and fitting services, frames, lenses, contact lenses, adjustments, and related follow-up care. The proposer must provide monthly capitated rates, a network listing of convenient service locations, participant cost information for non-covered services and upgrades, promotional-impact details, grievance administration, member brochures, a toll-free service number, utilization reporting, eligibility maintenance, and quality-control procedures. Coverage is effective January 1, 2027, with renewal years identified for January 1, 2028, and January 1, 2029. The proposal must include the required pricing, qualifications, service explanation, certifications, addenda acknowledgment, and other submission materials. Mandatory insurance requirements include statutory workers' compensation, employers' liability with $1,000,000 limits, and comprehensive general liability with a $1,000,000 combined single limit.
