Description
Palm Beach County is soliciting applications from qualified governmental and nonprofit entities to provide Ryan White HIV/AIDS Program Part A and Minority AIDS Initiative services for people with HIV. Applicants may propose one, several, or all eligible service categories, including early intervention, outpatient and specialty medical care, medical and non-medical case management, mental health, oral health, health insurance premium and cost-sharing assistance, substance abuse outpatient care, emergency financial assistance, food and home-delivered meals, medical transportation, legal services, health education and risk reduction, outreach, and referrals for health care and support services. Applicants must describe their prioritized populations, service model, expected outcomes, implementation plan, and budget, and must comply with payer-of-last-resort, eligibility, reporting, licensing, and federal grant requirements. The project term is 12 months beginning March 1, 2027, with options to renew for two additional 12-month periods through February 28, 2030. The estimated first-year funding levels are $6,176,102 for Part A services and $537,601 for MAI services. Applications must be submitted electronically through the County's NOFO submission website, and the Proposal Cover Sheet must be signed by an authorized officer and notarized. Required materials include the online application, cover sheet, submission checklist, internal control questionnaire, organizational and financial documentation, service-specific budgets and implementation plans, and applicable supporting policies, licenses, agreements, and certifications.
