Description
The Department of Veterans Affairs is soliciting firm-fixed-price, indefinite-delivery/indefinite-quantity contracts for community-based medical respite transitional housing for homeless Veterans. Contractors must provide safe housing, room and board, meals and snacks, laundry, hygiene products, medication monitoring and storage, transportation, and accommodations for Veterans with enhanced medical needs, including access for visiting home-health providers. Required supportive services include case management, therapeutic and rehabilitative services, individualized care plans, psychosocial assessments, weekly case-management meetings, structured group activities, assistance with benefits, employment, budgeting, permanent housing, and healthcare access. Contractors must maintain qualified staffing, 24/7 awake supervision, appropriate medical and behavioral-health personnel, facility licensing and safety compliance, accessibility, emergency plans, secure health-information storage, incident reporting, inspections, and detailed Veteran records. The contract includes a one-year base period and four one-year option periods, with pricing submitted as a daily rate per bed/Veteran.
