Description
Cook County Department of Facilities Management is soliciting services for fall protection anchorage load testing, suspended scaffolding davit load testing, annual visual surveys, certification, repairs and retesting of failed components, and related compliance reviews. The contractor must test anchorages and davit systems to specified 5,000-pound loads, provide required testing equipment, and perform work in accordance with applicable OSHA, ANSI, International Window Cleaning Association, and Chicago Municipal Code requirements. The scope also includes written load-test and condition-survey reports, equipment-use plan reports, and compliance audit reports for each facility. Reports must include roof plans, component identification, photographic documentation, certification status, maintenance recommendations, rigging-system evaluations, and potential modification costs; load-test certification must be provided by an Illinois professional engineer. The contract term is three years with one two-year renewal option. A minimum of three years' relevant company experience and three years' experience for assigned personnel are required, along with at least three client references and personnel certifications and resumes.
