Description
The Chicago Park District is soliciting bids from experienced firms to provide ongoing maintenance and repair for elevators and multiple types of accessibility equipment, including chair, platform, stage, vertical, LULA, wheelchair, and portable pool lifts throughout the district. The submission requires numerous forms and supporting materials, including a signature page, bid, economic disclosure statement and affidavit, Chicago business affidavit, self-performance affidavit, subcontractor form, sustainability statement, financial statement, legal actions disclosure, MBE/WBE schedules and certification letters, vendor references, bidder description, experience, staffing plan, state certificate of incorporation, insurance certificate, applicable Illinois or Chicago licenses, and a vendor references form.
