Description
Baltimore County is seeking qualified organizations to implement mental health case management services for eligible adults who have Medicaid or are uninsured and meet applicable medical-necessity and financial-eligibility requirements. Services include comprehensive assessment and reassessment, individualized care-plan development and revision, referrals and related activities, monitoring and follow-up, participant advocacy, benefits and entitlement assistance, substance-use screening and linkages, outreach, crisis response, and coordination with behavioral health, housing, medical, vocational, educational, and other community providers. Selected providers must offer both general and intensive levels of care, maintain qualified and trained staff, provide accessible community-based services with in-person care prioritized when appropriate, support urgent needs 24 hours a day, seven days a week, and maintain written policies, quality-assurance plans, outreach plans, training plans, and reporting processes. Proposals must include a technical proposal addressing organizational capacity, service delivery, staffing, focus-population access, system-of-care principles, implicit bias and systemic racism, program evaluation, and quality assurance, along with the required forms and supporting documentation. The resulting agreement is for one year with up to four additional one-year renewal options, and reimbursement will be accessed through Maryland's Public Behavioral Health System rather than grant funding.
