Description

Baltimore County Department of Health, Bureau of Behavioral Health/Local Behavioral Health Authority, is seeking one qualified nonprofit vendor to operate three related programs: Projects for Assistance in Transition from Homelessness (PATH), PATH State Match outreach, and a SOAR specialist program. Services include outreach to people experiencing homelessness or at imminent risk of homelessness who have serious behavioral health conditions; screening, enrollment, case management, referrals, housing assistance, benefits assistance, Continuum of Care participation, community presentations, and behavioral health service coordination. The SOAR component requires a full-time specialist to assist eligible individuals with SSI/SSDI applications, obtain records, prepare Medical Summary Reports, track cases, conduct provider outreach, and maintain a minimum 75% approval rate. The proposal must address program implementation, staffing and qualifications, outreach, quality assurance, emergency response, HIPAA compliance, nonprofit status, board documentation, references, and separate technical and price proposals. The three projects have stated annual compensation totaling $263,191, with separate budgets required for PATH, PATH State Match, and SOAR.
