Description
The City of Toledo Department of Public Services and Facility Operations is seeking bids for carpet replacement services at Eleanor Kahle. A Project Labor Agreement (PLA) is required for this project. Bids must be submitted electronically through the City of Toledo's PlanetBids portal, and confirmation of transmission from PlanetBids is required. Bid prices should be submitted with two or four decimal places, with only one price per line item accepted. Required documents for bid submission include Construction Project Paperwork, Line Items, Bid Guaranty (bid bond or certified/cashier's check), a Signed Contract by the Vendor, and a Tax Exemption Certificate. Bid Guaranty details are on the "Bid Information" tab; checks should be made payable to the "City of Toledo" and received by the Division of Purchases and Supplies by the due date. Bid bonds do not need to be submitted in original form. Failure to provide a bid guaranty will result in a non-responsive bid. Beginning March 13, 2026, certain documents only need to be submitted once annually for the duration of all City contracts, including Vendor Registration Form (if new), Business Tax Registration Form (if new), a notarized Child Support Affidavit, Vendor Utility, W-9, Certificate of Insurance naming the City of Toledo as additionally insured, Worker's Compensation Certificate, EEO Compliance, Debarment Certification, and Living Wage Affidavit (updated by February 1st). Project Paperwork includes Acknowledgment and Acceptance of Conditions Contract Specifications, Primary and Subcontractor Forms, Living Wage Contract Addendum, and Best Bid Criteria. Upon contract award, a Payment Bond, Performance Bond, and PLA (if applicable) will be required. Certified payrolls must be submitted two weeks after the start date and weekly thereafter. A pre-construction meeting with the Contract Compliance office is required before work begins. Prevailing Wages are required, and rates can be found on the City of Toledo or Ohio Department of Commerce websites. A local preference is noted per TMC Section 187.34.
