Description
Halifax Health is seeking a qualified general contractor to renovate the Quality Department on the second floor of the Fountain Building at its Daytona Beach campus. The work includes demolition and replacement of existing flooring; subfloor patching and preparation; installation of luxury vinyl tile flooring and wall base; painting of walls and door frames; replacement of closet doors; installation of owner-provided roller shades; required infection-control measures; daily housekeeping; and final construction cleaning. The department will remain occupied and operational during portions of the project, so the contractor must minimize disruption and comply with healthcare-facility policies, background screening, badging, and infection-control requirements. The bid package must include a signed cover letter, proposed scope, project approach, key personnel, three similar completed construction projects, at least three professional references from the past five years, a preliminary project schedule, schedule of values, billable rates, and all required attachments and certifications. Required supporting materials include the bidder's proposal, public-entity-crimes statement, bidder's certification, human-trafficking attestation, scrutinized-companies certification, foreign-country-of-concern attestation, subcontractor list if applicable, total project cost form, Bonfire construction questionnaire, business-registration and applicable-license verification, certificate of insurance, and completed IRS Form W-9. Award is based on the lowest responsive and responsible bid or the bid most advantageous to Halifax Health.
