Description
Palm Beach County Community Services Department is seeking an existing, commercially available Software-as-a-Service platform supporting digital client engagement, care coordination, recovery support, wellness, case management, and communication for behavioral health, substance use, public health, housing, and human-service programs. The County is not seeking development of a new custom application. The selected contractor must configure, customize as needed, integrate, implement, test, maintain, upgrade, and support the platform. Required capabilities include client-facing mobile tools, check-ins, appointment reminders, secure communication, telehealth support, resource navigation, Social Determinants of Health data, care-team workflows, case management, alerts, dashboards, analytics, interoperability through APIs and HL7/FHIR, security and privacy controls, onboarding, training, technical assistance, and ongoing support across multiple provider organizations.
