Description
The County seeks a provider to deliver integrated prevention and early intervention services countywide to children from birth through age five and their families or caregivers. Services include developmental and behavioral screening, assessment, and treatment for children; mental health screening, assessment, and referrals for caregivers; care coordination and closed-loop referrals; caregiver coaching and workshops; and multidisciplinary collaboration. The program must use Motivational Interviewing, implement or partner with a Healthy Steps site, and provide culturally and linguistically responsive services. Services may be delivered in person or virtually, and must reach rural areas as well as other parts of the county. The provider is responsible for program startup, implementation, maintenance, evaluation, data collection and reporting, and coordination with community-based organizations and the Commission-appointed support entity. The proposal must include organizational experience and staffing information, references, program approach and implementation plan, a linguistic and cultural diversity plan, cost and budget materials, and responses concerning the draft agreement and financial and organizational stability. The initial contract period is one year, with four one-year options.
