Description
The Utah Department of Health and Human Services seeks qualified providers to deliver time-limited peer support services to individuals and their families while they await Home and Community Based Services. The services are intended to help participants remain in their home and community-based settings and will result in contracts for qualified vendors. Responses must follow the instructions in the Invitation for Applications document and be emailed to the designated contact; questions must be submitted through the state's procurement system. Solicitation documents are available through the DHHS Purchasing site.
