Description
The State of Wyoming is seeking a contractor to provide technical assistance for Projects for Assistance in Transition from Homelessness (PATH). The scope includes connecting with current PATH subrecipients, identifying process improvements, developing streamlined referral pathways for mental health and substance use treatment in rural environments, and creating structured follow-up and ongoing support protocols for PATH-eligible clients. Deliverables include a comprehensive training document, one live virtual training, one separately recorded training, and a standardized procedure guide for continuity of care. The contractor must provide relevant work examples, a project work plan, staffing information and resumes, references, a budget narrative, and an all-inclusive price proposal. The estimated funding available is $10,152, and proposals exceeding that amount will not be considered. The virtual training and related documents must be approved by July 30, 2027, the virtual training completed by August 13, 2027, and all remaining deliverables provided by August 23, 2027.
