Description
The Northport Veterans Affairs Medical Center (VAMC) requires specialized cytopathology services with comprehensive testing for the evaluation of thyroid nodules. This includes testing for indeterminate cytology such as Atypia of Undetermined Significance (AUS) and Follicular Lesion of Undetermined Significance (FLUS). The requirement specifically includes cytopathology evaluation of fine-needle aspiration (FNA) samples, reflex molecular testing when cytology is indeterminate, and Next-Generation Sequencing (NGS) of DNA and RNA from thyroid nodule FNA material. The service must analyze thyroid-related mutations, gene fusions, copy number alterations, and expression alterations, delivering a single integrated positive/negative risk classifier compatible with current clinical management pathways and historical patient data. The VAMC currently utilizes the proprietary ThyroSeq(r) Genomic Classifier, and continuity of care requires compatibility with existing veteran molecular records and seamless integration into the established diagnostic workflow. Interested parties must demonstrate their ability to provide integrated cytopathology and reflex molecular NGS in a single laboratory workflow, perform DNA and RNA sequencing covering specific alterations, deliver results comparable to existing ThyroSeq(r) data, and meet required turnaround times. This is a notice of intent to award a sole source contract.
