Description
Community Healthcore seeks a vendor to provide a fully managed Chronic Care Management program for patients receiving primary care and/or behavioral health services. The scope includes identifying eligible patients, verifying coverage, conducting approved outreach and enrollment, documenting consent, recurring patient engagement, person-centered care planning, medication review, barrier assessment, care coordination, clinical escalation, quality assurance, and monthly billing support. Software-only and staff-augmentation-only proposals do not meet the required scope; the vendor must provide and supervise the staff needed for its assigned functions. The vendor must support both primary care and behavioral health populations and work within the Center’s Credible and NextGen electronic health records. Proposals must address implementation, staffing, clinical workflows, documentation and EHR alignment, billing integrity, pricing, performance reporting, and information security. All personnel and data hosting or processing must be located in the United States. Respondents must provide three comparable client references and a detailed response to the six requested evaluation categories. The contract is scheduled to end August 31, 2027, with three possible one-year extensions.
