Description
The City of Houston's Houston Health Department is soliciting bids for 31 new Visa prepaid gift cards, each with an exact face value of $100, for eligible participants who complete maternal interviews as part of the Fetal and Infant Morbidity Review Program. The cards must be fully funded, ready for use upon receipt or securely activatable, accepted wherever Visa prepaid cards are normally accepted, and include balance and customer-service instructions. Cards must have an expiration period of at least three years from purchase, with five years preferred, and may not impose activation, maintenance, dormancy, monthly service, or user fees that reduce the participant's $100 value. The vendor must securely package and deliver the cards, provide delivery documentation, disclose any separate administrative or shipping charges, and promptly replace defective or unusable cards attributable to the vendor. The bid must include itemized pricing and proposed delivery timing.
