Description
The Indian Health Service is soliciting quotations for six commercial-grade fitness machines: five multi-station strength-training units and one commercial stair climber with an LED console. Brand-name-or-equal products are acceptable if they meet the stated salient functional and performance characteristics. Quantities are firm, and pricing must be FOB destination and include packaging, handling, shipping, and delivery. The contractor must provide inside delivery, assembly, placement, leveling, installation, safety and operational verification, debris removal, basic staff orientation, and manufacturer manuals and warranty documentation. Performance begins upon award, with completion required within 30 to 60 calendar days unless otherwise established in the award. Competition is limited to firms that qualify as both Indian Economic Enterprises and small businesses, and the offeror must submit the Indian Economic Enterprise Representation Form, maintain active SAM registration and a current UEI, and certify authorized-reseller and warranty support. Award will use a lowest-price technically acceptable approach, with past performance evaluated as acceptable, unacceptable, or neutral.
