Description
Cedar Hill Independent School District is soliciting proposals from qualified vendors to provide athletic and physical education equipment, apparel, goods, and related services. The solicitation is intended to establish a list of pre-qualified vendors that district campuses and departments may use for future purchases; multiple vendors may be awarded, and no pricing is requested at this time. The district may also permit participating educational entities and purchasing cooperatives to use resulting contracts under the same terms, conditions, and pricing. Responses must be submitted online through the electronic bid system; emailed, hard-copy, and faxed responses will not be accepted. Required submission materials include a certificate of insurance, completed W-9, Form 1295, completed conflict-of-interest form, ACH form materials, and the pre-service affidavit for educational entities. Vendors must also complete the required electronic certifications and provide three references for comparable goods or services, preferably for Texas school districts. The agreement is for three years, with two successive one-year extensions by mutual consent, for a maximum term of five years.
