Description
The Connecticut Department of Mental Health and Addiction Services is soliciting proposals for Homeless to Housing Services programs. Eligible private nonprofit organizations and Connecticut municipalities will provide interdisciplinary teams to support people experiencing unsheltered or sheltered homelessness in locating and sustaining safe, permanent housing. Services must follow the consumer from outreach and shelter through housing stabilization, with minimal staff handoffs. The scope includes outreach and engagement, coordination with shelters, Coordinated Access Networks, behavioral health and employment or Social Security benefit providers, eligibility verification, housing location and lease negotiation, housing assessments, individualized housing sustainability plans, housing stabilization, and required data collection through HMIS and DMHAS systems. Proposers must demonstrate at least five years of relevant experience, identify the Coordinated Access Network region they will serve, and submit an implementation plan, staffing plan, organizational chart, technology and data-management approach, financial information, annual budgets for the three-year term, and required state forms and certifications. Up to five lead-agency contracts may be awarded per Coordinated Access Network, with total funding anticipated at $22,770,513 annually and approximately 3,300 statewide service slots.
