Description
Stanislaus County seeks one contractor to deliver its Homeless Assistance Program (HAP) and Housing Support Program (HSP) for eligible CalWORKs participants. HAP services include coordinating temporary motel shelter, landlord outreach, housing leads, case management, housing counseling, and assistance with rental and utility-related needs. HSP services include housing navigation, case management, emergency shelter coordination, landlord engagement, referrals to community resources, and participant data entry and reporting through HMIS. The contractor must be able to transition participants between the programs and serve bilingual participants. The work also includes staffing two case managers at a family housing facility for at least 20 hours per week each, maintaining motel arrangements for after-hours referrals, and submitting quarterly outcome scorecards and budget narratives. Available funding is stated as $90,000 for HAP and $314,000 for HSP, totaling no more than $404,000 for the initial agreement period. The County anticipates an initial one-year agreement and may extend it year to year for up to two additional years.
