Description
Iredell County seeks a qualified provider to deliver on-site physical therapy for eligible employees, retirees, and dependents. Services include initial and periodic evaluations, individualized plans of care, direct treatment such as orthopedic rehabilitation, post-operative therapy, chronic pain and injury management, manual therapy, and therapeutic exercise. The provider must coordinate care with referring physicians and the County’s Employee Wellness Clinic, maintain compliant clinical records, provide monthly itemized invoices, and ensure staffing coverage during absences. The County makes its existing treatment space and listed equipment available at no cost; the provider supplies any additional equipment and materials needed. Proposals should describe the provider’s qualifications and comparable experience, clinical staffing and coverage model, implementation and transition plan, scheduling and communications, reporting and performance guarantees, and pricing. Respondents must complete the attached E-Verify and Iran Divestment Act certifications, provide a detailed pricing proposal using Exhibit A, and submit the other requested proposal responses, references, and the company’s standard proposed contract.
