Description
Chemung County is soliciting a contractor to provide comprehensive therapy and rehabilitation services at its 200-bed nursing facility, including physical therapy, occupational therapy, and speech-language pathology. Services include therapist staffing, clinical oversight, quality assurance, Minimum Data Set and care-plan documentation, PDPM and case-mix management, Medicare and Medicaid reimbursement support, billing, reporting, denied-claim appeals, and participation in facility quality-assurance and Medicare Triple Check processes. The provider must supply appropriately licensed personnel, including a qualified Director of Therapy, and support the facility's rehabilitation program across applicable payer sources. The contract is for one year beginning January 1, 2027, with up to two additional one-year extensions by mutual agreement. Proposers must submit qualifications, key-personnel information, at least five comparable client references, service approach, Medicare and Medicaid experience, financial information, pricing, and any proposed additional services. Annual billing is not to exceed $1.3 million. The proposal package also requires the specified certifications and forms, including the Proposal Submission Form, Vendor Responsibility Form, Non-Collusive Proposal Certificate, Waiver of Immunity, Iranian Energy Divestment Certification, and Sexual Harassment Certification.
