Description
The New York State Office of Mental Health is seeking a firm or individual to provide physical therapy services for up to 80 hours per month over a five-year term. Services will support adult and children/youth programs and residents of the Secure Treatment and Rehabilitation Center, with up to 20 hours per month allocated to adult and children/youth patients and up to 60 hours per month allocated to the secure treatment program. Regular service hours are generally Monday through Friday from 7:00 a.m. to 4:00 p.m., with a flexible schedule; overtime and on-call duty are not permitted. The scope includes managing referrals, patient screening and evaluations, treatment planning and delivery using physical therapy modalities, patient assessments, discharge summaries, consultation and training for staff, clinical supervision of physical therapy assistants and paraprofessionals, treatment documentation, treatment-team participation, and related duties. The contractor must provide appropriately licensed and certified physical therapists, comply with facility, professional, health, background-check, confidentiality, and patient-safety requirements, and submit the required bid forms, certifications, references, insurance and licensing documentation. Award will be made to the responsible and responsive bidder offering the lowest total bid amount while meeting mandatory requirements.
