Description
The County seeks a qualified, New York-registered speech-language pathologist to provide assessment and treatment for clients with speech, voice, language, or swallowing difficulties. Services include evaluations, individualized plans of care, direct therapy, client and family instruction, consultation with public health staff and authorized practitioners, progress monitoring, and clinical documentation. Services may include home visits or two-way audio-video telecommunication, as appropriate to the department's needs. The initial contract term is three years from the date of award, with two optional one-year extensions by mutual agreement. Proposers must provide hourly rates for the listed contract years and describe any additional costs. The RFP evaluates technical response, experience, project management and support, service quality and cost. Required insurance includes workers' compensation, disability, automobile, general liability, and professional liability coverage.
