Gloucester County Improvement Authority seeks licensed New Jersey physical therapists, occupational therapists, and speech therapists to provide rehabilitation services for residents of Shady Lane Home. Services include resident assessments at admission, readmission, quarterly intervals, and significant changes in condition; care planning; medical-record maintenance; fall screening within 72 hours; participation in mandatory resident-care conferences, weekly falls meetings, and quarterly rehabilitation-team meetings; restorative-program development; equipment management; and coordination with nursing, rehabilitation, and MDS staff in compliance with applicable local, state, and federal regulations. The selected provider must be able to use the Sigma Care System for order writing and documentation, use facility-supplied equipment, maintain equipment records, label braces and splints with residents' names, provide current malpractice-insurance documentation, and notify facility leadership of service or staffing changes affecting residents. Proposals must include a fee and cost proposal identifying any retainer and the services covered; travel expenses will not be reimbursed.
