Description
The Town of Fairfield, on behalf of Fairfield Public Schools, seeks qualified firms to provide occupational therapy (up to 25,000 hours annually) and physical therapy (up to 10,000 hours annually) for students ages 3–22, including services during the school year and extended school year programs. Services must be delivered by appropriately licensed and qualified therapists in school and home settings; teletherapy is not offered. The scope also includes participation in planning and placement team meetings, collaboration and consultation with school staff, reporting and Medicaid documentation, staffing continuity and substitutes, and related professional support. The initial contract term is two years, beginning on or about July 1, 2027, with three optional one-year renewals. Proposals must include a work plan, staffing and screening approach, relevant experience and references, staff retention and turnover information, a sample contract and invoice, and pricing in the designated pricing table. The solicitation also requires completion of its online response fields and specified acknowledgments; an exceptions form is optional when applicable. A certificate of insurance is due from the awarded proposer before contract execution, not with the proposal.
