Description
The State of Utah Division of Facilities Construction and Management is requesting proposals from DFCM-approved special inspection firms, material testing laboratories, and special inspectors for the Utah State University Animal Science Renovation and Expansion project in Logan, Utah. The work includes special inspections and materials testing for the substantial renovation of a historic Animal Sciences Building and construction of a new three-story steel and concrete addition with a basement, including structural, site, envelope, mechanical, electrical, and interior work across multiple bid packages. The proposal must address the contractor’s qualifications, project team and certifications, management plan, organizational chart, comparable project experience, inspection scheduling approach, and the team’s one-way distance from its dispatch location to the project site. Sections One through Three must be combined into one PDF of no more than 10 pages, with a minimum 12-point font, 8.5-by-11-inch pages, and margins of at least 0.5 inch. The DFCM Termination or Debarment Form and, when applicable, the DFCM Past Performance References Form must be uploaded separately. The estimated inspections budget is $192,438.
