Description
Genesee Health System is establishing agreements with multiple qualified providers to expand its provider network and address the community need for after-hours Applied Behavior Analysis services. Services are for children and young adults under 21 who reside in Genesee County, have Autism Spectrum Disorder, are eligible for Medicaid, and are determined able to benefit from Applied Behavior Analysis. Providers must offer services after 4:00 p.m. on weekdays and/or during weekday evenings and weekends, in clinic-based settings, home-based settings, or both within Genesee County. Responses must address geographic readiness, evening and weekend capacity, staffing and recruitment, referral readiness, continuity of care, and quality assurance. Applicants must submit the required forms and supporting organizational documentation, including entity documentation, credentialing and disclosure materials, an organizational chart, litigation disclosure, W-9, certificate of insurance, and two years of financial reporting. Multiple provider agreements are anticipated, subject to credentialing and privileging approval.
