Description
Yuba County Health and Human Services Department is seeking consulting services to support the County’s second CalWORKs Outcome and Accountability Review continuous quality improvement cycle. The scope includes project management, CQI planning, program evaluation, data collection and analysis, demographic and equity analysis, dashboards and graphics, stakeholder engagement, focus groups, surveys, County Self-Assessment support, peer review, System Improvement Plan development, and implementation support. The initial contract term is two years, with two optional one-year extensions. Required proposal components include an application, statement of experience, proof of insurance, references from two similar-service agencies within the last five years, a scope statement, a separate two-year cost proposal, SAM.gov entity verification, and several required certifications and declarations. Proposals must be submitted through the procurement portal; hard-copy proposals are not accepted.
