Description
Schenectady County Senior and Long Term Care Services is seeking an entity to provide efficient, cost-effective home-delivered meals and operate three congregate meal sites throughout Schenectady County. Services are for adults age 60 and older, and, when appropriate, spouses of eligible participants residing with them. The program targets individuals with the greatest social and economic needs, including low-income, underserved and underrepresented older adults, rural residents, people with limited English proficiency, and those at risk of institutional care. The proposal must address meal preparation and delivery, congregate meal-site operations, food safety, nutrition, staffing, participant services, recordkeeping, reporting, quality assurance, emergency continuity, and transition planning. The contract is for one year with four one-year renewal options. Proposals must include the required County forms and certifications, proposal materials, three references, insurance certificates, and a USB/flash drive copy.
