Description
The City and County of San Francisco Department of Public Health is soliciting proposals for food and nutrition services for low-income San Francisco residents living with HIV. The selected provider must offer grocery distribution, prepared hot and frozen meals, hot meals at its facility, frozen-meal pickup, and home-delivered meals for clients with complex health or mobility needs. Grocery services must include nutritious staples, fresh foods, special-diet options, and no-cook bags for clients without cooking facilities. Meals must accommodate dietary and medical needs including vegetarian/vegan, gluten-free, renal, diabetic, mechanical-soft, bland, and low-fat/no-dairy diets. The scope also requires nutrition education, individual counseling, cooking demonstrations, nutrition assessments, case management, client eligibility and service-data reporting, client satisfaction surveys, and compliance with food-safety requirements. A Registered Dietitian must provide nutrition counseling. Proposers must demonstrate specified HIV-related food-service experience, consortium experience, and the ability to employ or recruit a Registered Dietitian. The anticipated contract term is five years with five one-year options, and the estimated annual amount for year one is $1,656,637.
