Description
The Town of Greenwich is requesting proposals for a pulmonary rehabilitation program and oxygen supplier for The Nathaniel Witherell, a 202-bed skilled nursing facility. The selected contractor will provide clinical evaluations, care-plan implementation, respiratory therapy personnel, clinical management, coverage for absences, staff supervision and training, physician-order execution, patient education, respiratory records, oxygen equipment monitoring, and related pulmonary rehabilitation support. The scope also includes supplying and maintaining oxygen equipment and respiratory supplies, including concentrators, liquid oxygen, portable oxygen units, E tanks, CPAP and BiPAP equipment, cough-assist devices, air compressors, suction machines, ventilators, and associated setup or rental services. Pricing must include regular, weekend, holiday, overtime, and implementation competency-training rates. Proposals must include the completed Reply Sheets, pricing, qualifications, references, required insurance documentation, and other requested vendor information.
