Description
The City of Kentwood is soliciting Statements of Qualifications from licensed physical therapy providers to support its Police Department. The selected provider will deliver on-site physical therapy, injury prevention, movement and functional assessments, rehabilitation, wellness programming, return-to-duty conditioning, injury consultations, wellness clinics, training sessions, and return-to-work support for law enforcement personnel. Clinicians must hold a Doctor of Physical Therapy degree, maintain an active and unrestricted Michigan physical therapy license, carry professional liability insurance, and have at least three years of relevant clinical experience. Respondents must describe their qualifications, comparable public-safety experience, project team, service approach, on-site capabilities, and provide at least three references for comparable services performed within the past five years, along with a fee schedule. The agreement has an initial one-year term with up to four additional one-year options; services are expected to average approximately two hours on site twice monthly, with additional services available as needed.
