Description
The County seeks a contractor to assess organizational readiness for integrating Community Health Workers (CHWs) and develop standardized curriculum and training materials. Work includes needs assessment; facilitator and participant resources; presentations, activities, and assessments; implementation tools such as action plans, checklists, workflows, and sample policies; technical assistance resources; and pilot testing followed by revisions. Final deliverables must be editable electronic files suitable for reuse across organizational settings. Services may be performed virtually. Funding is capped at $62,000, and the anticipated performance period runs from contract award through June 30, 2027, with an option to renew for up to one additional year.
