Description
The City of Blue Springs seeks a qualified contractor to remove and legally dispose of existing flooring, adhesives, setting materials, and debris in an approximately 620-square-foot City Hall work area; prepare the exposed concrete subfloor; and install City-selected tile. The work includes setting materials, grout, tile wall base, transitions, trims, sealants, incidentals, protection of adjacent finishes and equipment, debris removal, and final cleanup. Tile materials must be priced using a $5.00-per-square-foot allowance, with any excess cost handled as an additional charge to the City. Proposals must include contractor qualifications, licenses and certifications, key personnel, relevant project experience and references, an itemized cost schedule, a detailed completion schedule, proposed materials and equipment, and pricing compliant with Missouri prevailing-wage requirements. The solicitation includes Forms A through D for pricing, experience and references, disclosures, and proposal validity/commitment to contract. Two copies of the original signed proposal are required. Insurance, affidavits, E-Verify documentation, and a W-9 are required only from the selected contractor during contract authorization, not with the initial proposal.
